Location: The Anthony School Field/Ohio Park

Acknowledgment of Risk and Release of Liability

​

I, the undersigned, whether as a participant or as the parent/guardian of the participant named below, in consideration of participating in scheduled activities and the use of The Anthony School Ohio Park, hereby voluntarily agree to assume full responsibility for any risks or loss, personal injury, including death, or damage to property that may be sustained by myself or any players I register, as a result of participating in the scheduled activities at the aforementioned premises.

Agreement to Hold Harmless, Indemnify, and Release from Liability

I agree to release, hold harmless, and indemnify KICKBALL ARKANSAS LLC, also known as Kickball Arkansas and/or Kickball World, The Anthony School, located at 7700 Ohio Street, Little Rock, Arkansas, and any partner coaches, companies, or field owners involved in the provision of activities at this location and any other locations used now or in the future, from any and all liability, actions, claims, demands, expenses (including attorney’s fees), and damages whatsoever, whether foreseen or unforeseen, that I, or any players I register, might now have or may acquire in the future, related to any loss, damage, injury, or death, sustained by me or said players, or to any property belonging to me or said players, accruing from any accident causing injury or loss.

Entire Agreement

This Release constitutes the entire agreement between The Anthony School and KICKBALL ARKANSAS LLC, concerning my participation and that of any players I register. This agreement is made as a condition precedent to our participation in scheduled activities. Should any portion of this Release be determined invalid, voidable, or unenforceable for any reason, such portion shall be severable from the remaining portions herein and shall not affect the remaining provisions of this Release. This Release is binding upon my family members, heirs, assigns, and executors.

By checking the box below, I acknowledge that I have fully read, understood, and agree to the terms of this Release. I understand that by check the box below, I am giving up substantial rights, including my right to sue.

 

This Release covers the participation times/dates that have been designated by the supervisor of the team(s) and approved by the head of The Anthony School. This document is legally binding and affects your rights.
